The number of inquiries into mental health tragedies is increasing rapidly. Within these inquiry reports there are important findings, together with recommendations for improving practice in the delivery of services to mentally disordered offenders in the community. However, the reports are published in an ad hoc and piecemeal way with the consequence that they are often difficult to obtain.
